What the studies show about insulin resistance, glucose metabolism, and everyday brain fog — and where the evidence is still catching up.

"Brain fog" doesn't have a formal clinical definition, but people consistently describe it the same way — trouble focusing, slower recall, a mental haze that doesn't map to any single diagnosis. Across recent research, one factor comes up again and again: how well the body regulates blood sugar and insulin.

The brain depends on glucose for nearly all of its energy. When the body's cells become less responsive to insulin — the core feature of insulin resistance — it raises a natural question researchers have started pursuing: does the brain experience a localized version of that same energy-delivery problem?

What the Research Shows

A 2023–2024 human study on peripheral insulin resistance found that people with higher insulin resistance, even without a diabetes diagnosis, showed reduced cerebral glucose metabolism and measurably impaired working memory performance compared to more insulin-sensitive participants.

Separately, longitudinal research has linked insulin resistance in otherwise metabolically healthy people to a greater risk of later cognitive decline, and to reduced glucose uptake in specific brain regions involved in memory and executive function, including areas of the prefrontal cortex. Researchers describe altered cerebral glucose metabolism as one of the more consistently observed factors connecting metabolic health to age-related cognitive change.

Much of the research on specific botanical compounds — including berberine, a widely studied plant alkaloid used traditionally for blood-sugar support — comes from animal models of diabetes-related cognitive impairment rather than large human cognitive trials. In these animal studies, berberine has been shown to improve glucose uptake in brain tissue and reduce markers of neuroinflammation alongside its well-documented blood-sugar effects. Human research on berberine has focused mainly on metabolic markers like blood glucose and cholesterol, not cognitive outcomes directly, so this connection remains promising but not yet confirmed in people.

What This Means for You, Practically

The current, honest takeaway is that metabolic health and brain performance appear genuinely connected, but the strongest evidence today points to lifestyle basics: balanced meals that avoid large blood sugar swings, regular physical activity, adequate sleep, and, when appropriate, medical management of diagnosed insulin resistance or diabetes. These same habits are independently associated with better cognitive outcomes in long-term studies.

Botanical ingredients traditionally used for blood-sugar support are a reasonable area of ongoing research interest, but they should be viewed as a complement to — not a replacement for — those fundamentals, medical care, and, where relevant, prescribed treatment for diagnosed metabolic conditions.
